Dentist vs. Oral Surgeon
for Dental Implants

Choosing the right provider for your dental implant procedure is one of the most important decisions you’ll make. Understanding the differences in training, surgical expertise, and scope of practice between a general dentist and an oral surgeon helps you make the best choice for your long-term outcome.

Understanding your options

Who should place your dental implants?

Both general dentists and oral surgeons can place dental implants — but their training, surgical experience, and ability to manage complex cases differ significantly. For many patients, especially those with straightforward anatomy and good bone volume, either provider may be capable. For patients with more complex needs, the distinction matters a great deal.

At DFWOMS, our board-certified oral and maxillofacial surgeons bring years of specialized surgical residency training specifically focused on the mouth, jaw, and face — including the full spectrum of implant procedures, from single-tooth replacements to full-arch rehabilitations with bone grafting and sinus lifts. Understanding what sets them apart helps you make a confident, informed decision.

Training & expertise

How their training differs

DDS
General Dentist

General dentists complete four years of dental school and are trained to manage a broad range of oral health needs — from routine cleanings to restorative work. Many dentists pursue additional courses or continuing education in implantology. However, the depth of surgical training, the volume of implant cases, and the ability to manage complex scenarios can vary widely from one dentist to another.

OMS
Oral & Maxillofacial Surgeon

After dental school, oral and maxillofacial surgeons complete an additional four to six years of hospital-based surgical residency — training extensively in surgery, anesthesia, and the management of surgical complications. This residency includes thousands of procedures across the full scope of oral, jaw, and facial surgery. Implant placement is a core competency, practiced alongside bone grafting, sinus lifts, facial trauma, and reconstructive procedures.

Where the difference matters most

Four areas where an oral surgeon has a clear advantage

Complex cases — bone grafting, sinus lifts, ridge augmentation, and full-arch rehabilitation (All-on-4) require surgical expertise that goes well beyond basic implant placement
Anesthesia options — oral surgeons are trained to administer IV sedation and general anesthesia in an office setting; most general dentists offer only local anesthesia or nitrous oxide
Complication management — surgical complications such as nerve proximity, sinus perforation, or infection require the rapid, skilled response that surgical residency training provides
3D surgical planning — oral surgeons routinely use cone beam CT scans and computer-guided surgical planning tools that allow for sub-millimeter implant placement accuracy
Hospital privileges — for the most complex cases, oral surgeons hold privileges at hospitals and surgical centers, allowing treatment of patients who cannot be safely managed in an office setting
Medical complexity — patients with significant medical histories, on blood thinners, or with systemic conditions benefit from the medical-surgical training that oral surgeons receive during residency

Which is right for your case?

A practical guide to choosing your provider

DDS
A general dentist may be appropriate when…

Your case is straightforward: healthy gums, adequate bone volume, no grafting required, and a single tooth or a small number of implants in favorable positions. You have an established relationship with a dentist who has significant implant experience and volume. No IV sedation or general anesthesia is needed. Your medical history is uncomplicated.

OMS
An oral surgeon is the better choice when…

Your case requires bone grafting, socket preservation, sinus lifting, or ridge augmentation. You are considering full-arch rehabilitation (All-on-4 / BiteLock™). You prefer IV sedation or general anesthesia for comfort. You have a complex medical history. Your implants are near nerves, sinuses, or other critical structures requiring 3D imaging guidance. You want the highest level of surgical precision and complication management.

Frequently asked questions

Common questions about choosing your implant provider

Not always — for truly straightforward cases with ideal bone volume and no complicating factors, an experienced general dentist with significant implant training may perform the procedure well. However, for any case involving bone grafting, sinus lifts, full-arch rehabilitation, complex anatomy, or the need for IV sedation, an oral surgeon’s specialized training provides a meaningful advantage in both safety and outcomes.

Yes. You do not need a referral to schedule a consultation at DFWOMS. Many patients contact us directly after being told by their dentist that they need a more complex procedure, or simply after deciding they want a surgical specialist to handle their implant. Our team will evaluate your case, review imaging, and provide a clear recommendation — including coordination with your restorative dentist for the crown phase.

Oral surgeons can administer IV sedation and general anesthesia, perform complex bone grafting and sinus lift procedures, manage surgical complications as they arise, use 3D imaging and computer-guided surgical planning for precise placement, and treat patients with significant medical histories that require hospital-level oversight. They also perform full-arch rehabilitations such as All-on-4 — a procedure that requires simultaneous extraction, grafting, and implant placement in one surgical session.

Typically, the oral surgeon handles the surgical phase — implant placement, bone grafting, and healing — while your restorative dentist fabricates and attaches the final crown, bridge, or prosthesis. DFWOMS coordinates directly with your restorative dentist throughout the process to ensure seamless, integrated care. If you do not have a restorative dentist, we can refer you to one in our network.

Coverage varies by plan. Some dental insurance plans cover a portion of implant surgery, particularly the surgical placement fee. We accept most major dental insurance providers and Medicaid at all three of our DFW locations, and we offer CareCredit and LendingClub financing. Our team will review your benefits before your procedure so you have a clear picture of your costs — contact us before your consultation.

The best way to find out is through a consultation with our surgeons. We will evaluate your bone volume with 3D imaging, review your medical history, and assess the position and anatomy of the implant site. If your case is straightforward, we will tell you honestly. If it requires bone grafting, sinus augmentation, or other surgical steps, we will explain exactly what is needed and why. There is no obligation, and the information you gain from the consultation is valuable regardless of where you ultimately proceed with treatment.
