Complex & Surgical Tooth Extractions in Dallas–Fort Worth
Not all teeth come out easily. When a routine extraction isn’t enough, DFWOMS’s board-certified oral and maxillofacial surgeons provide precise, compassionate surgical tooth extraction care at our three locations in Irving, Mesquite, and Ennis.
When a routine extraction isn’t enough
A surgical tooth extraction is a more involved procedure than a simple extraction. It is required when a tooth cannot be removed with forceps alone — typically because it is impacted, broken beneath the gumline, surrounded by dense bone, or positioned near critical structures such as nerves or sinuses.
At DFWOMS, our board-certified oral and maxillofacial surgeons use advanced technology and precise surgical techniques to minimize trauma to surrounding tissues, reduce post-operative discomfort, and promote a smooth, faster recovery. From consultation through post-operative care, every step is handled with clinical excellence and genuine compassion.
When is a surgical extraction needed?
Your dentist may refer you to an oral surgeon — or you may come to us directly — when one or more of the following conditions apply:
Every complex extraction at DFWOMS begins with a 3D Cone Beam CT scan to map the exact position of the tooth, its roots, and all adjacent structures. This allows our surgeons to plan the safest extraction path — minimizing trauma, reducing recovery time, and preventing complications such as nerve damage or sinus involvement.
What to expect, step by step
We begin with a thorough evaluation of your oral health, medical history, and the affected tooth using a 3D Cone Beam CT scan. Our surgeons assess the complexity of the case, identify any nearby structures that require special care, and explain all your options clearly before moving forward.
We offer a full range of anesthesia and sedation options — from local anesthesia for straightforward surgical cases to IV sedation or general anesthesia for more complex situations or anxious patients. All protocols are opioid-free. We will recommend the best option for your comfort and safety.
Using precise surgical instruments and the guidance of our pre-operative imaging, our surgeons carefully remove the tooth — sectioning it if necessary to minimize the amount of bone disturbance. The goal is always to achieve a clean, controlled extraction while protecting surrounding tissues.
After removal, we clean the socket and, when appropriate, place a socket preservation graft to maintain bone volume for a future implant or prosthetic restoration. This step can prevent bone loss and simplify future treatment if tooth replacement is planned.
You will receive detailed aftercare instructions before leaving our office. Our team is available to answer questions during your recovery, and we schedule follow-up appointments to ensure proper healing. If tooth replacement is needed, we will coordinate next steps with your restorative dentist.

Common questions about surgical tooth extractions
A simple extraction is performed on a fully visible, fully erupted tooth that can be loosened and removed with dental forceps under local anesthesia. A surgical extraction is required when the tooth is impacted, broken, deeply decayed, or otherwise unable to be removed with standard instruments. It typically involves an incision in the gum tissue and may require sectioning the tooth or removing a small amount of bone.
The procedure itself is performed under anesthesia or sedation, so you should feel no pain during surgery. Some soreness and swelling is normal in the days following the procedure. Our opioid-free post-operative protocol manages discomfort effectively without narcotics. Most patients return to normal activity within a few days, though recovery varies depending on the complexity of the case.
In most cases, yes — leaving a gap after extraction can lead to bone loss, shifting of adjacent teeth, and bite problems over time. DFWOMS can discuss tooth replacement options at your consultation, including dental implants, which are the most natural and long-lasting solution. We also offer socket preservation at the time of extraction to maintain bone volume and simplify future implant placement.
If you are receiving IV sedation or general anesthesia, you will be asked to fast — no food or drink — for a specific period before your procedure (typically 6–8 hours). For procedures under local anesthesia only, fasting is generally not required. Our team will give you precise pre-operative instructions at your consultation.
Coverage depends on your specific plan. We accept most major dental insurance providers and Medicaid at all three of our locations. We also offer CareCredit and LendingClub financing options. Our team will review your benefits before your procedure so there are no surprises — contact us before your consultation.
We welcome referrals from general dentists across the Dallas–Fort Worth area and work closely with referring offices throughout the process. If your dentist has advised a referral, you can contact us directly to schedule your consultation. We will communicate with your dentist before and after the procedure to ensure seamless, coordinated care.
